Prelex / Presbyopic LASIK (After 40 Years)

It is a surgical procedure for persons over age of 40 yrs who want freedom from glasses. In this procedure we remove the natural lens from the eye and implant a new IOL, either monofocal or multifocal. In monofocal IOL distance vision glasses are eliminated whereas in multifocal IOL both instant and near vision glasses are not needed hence giving complete freedom from glasses.
It is a US FDA approved procedure.

This LASIK procedure is for persons having near vision glasses with or without distant vision glasses. With PAC LASIK a person can get independence from glasses for both near and distant vision. In PAC LASIK we create multifocality of the cornea. So a person can see both distant as well as near objects clearly without glasses. Cataract Surgery - Multifocal IOL

Phaco-emulsification (no need to wear glasses for both distance and near vision) is the latest technique in the fast developing world of cataract surgery. The incision is very small, hence no sutures are required and rehabilitation is quick and fast. The distortion of cornia is also minimal. In this surgery with the help of the machine the opaque lens is liquefied and then sucked out.  Instead of our natural lens, an artificial  intra ocular lens is placed. Usually the size of the IOL optic is 6 mm. The newer generations of IOLs are made from a flexible material & hence can be folded & injected into the eye with the help of injector. Through the sub 3 mm injection & hence the advantage of the small incision are maintained. The lens then unfolds inside the eye taking the place of the natural lens which has been removed. This surgery can be done under topical anesthesia in which case no injection is required to anesthetize the eye. Thus there is no need of bandaging the eye after surgery, so it is also called NO PAD, NO INJECTION cataract surgery.           
Topical anesthesia is benefical as it hastens visual recovery and there is no pain of injection and no eye ball manipulation as in case of peribulbar anesthesia. So topical anesthesia leads to instant visual recovery and no discomfort of injection anesthesia.
     
Monofocal Lenses      
 
Normal lenses provide either sharp distance vision or near vision and they are called monofocal lenses. Therefore the patient has to depend upon glasses for either near or distance vision. Blue bolcks IOLs, Toric IOL, Hydrophobic IOL & Hydrophilic IOL, Silicon IOL are other thpes of IOL.

What is Cataract?
Usually in normal eyes our lens is clear and transparent, but when cataract develops it becomes cloudy and opaque. So the light is not focused on the retina through lens which leads to blurring of vision.

How Cataract develops?
Most commonly cataract develops due to the aging process taking place in our lens. It is known as Senile Cataract.     

What are the Symptoms of Cataract ?
Gradually increasing blurring of vision is the most common symptom of cataract. Some people may have changes in the brightness of the seen objects. Glare may be experienced by some, specially at night. Difficulty in driving at night is common due to this. This is because of breaking up of light rays due to the cataract. Some people may have complaints of seeing rings or halos around lights. However, Cataract never causes pain, itching or redness.

Can Cataract be prevented?
No, till date no preventive measure is found for the Cataract though research continues. It is an age-related change. No drugs, diets or medicine can delay or cure the Cataract. However, wearing a sun-glass that blocks the UV rays can slow down the process of development of Cataract.

What exactly is done in Cataract Surgery?
Under an operating microscope and with Phaco-emulsification machine a small cut is made in the eye and opacified lens is liquefied and removed and an Intraocular Lens made of silicon or acrylic is placed in place of natural lens.

Will Surgery improve the vision?
Over 95% of Cataract surgery will improve the vision. While pre existing associated condition like glaucoma, diabetes, retinal condition etc may limit the vision even after surgery. Yet, Cataract surgery may still be worthwhile and necessary.

When should I Plan my surgery?
Under ordinary circumstances cataract surgery is not an emergency situation except in a few rare instances. Therefore in most of the cases the choice of undergoing surgery largely depends upon patients. In the past, because of unavailability of phaco emulsification technique surgeons usually waited till the time cataract became mature or ripe, but modern technique advances have now made possible to perform cataract removal at early stage. When it interferes with your day to day work, it is time for surgery. However, it is advisable to have cataract operated early before it becomes hard. An Ophthalmologist can give proper guidance.

What are the surgical techniques ?     
All cataract extractions are performed under an operating microscope under sterile conditions. The different techniques in use now for cataract extraction are :-   

  • Conventional ECCE ( Extra Capsular Cataract Extraction)
  • Stitchless Small incision cataract surgery
  • Phaco Emulsification

How long does one have to stay in the Hospital after the surgery ?      
 It is an Out Patient Surgery. No stay is required. Patient will be discharged immediately after the surgery.

What post operative care is required ?    
 After surgery you should be able to resume most activities almost immediately. You will be cautioned against lifting heavy weights and strenuous exercise to avoid putting unnecessary strain on the tiny structures as your eye heals.

The word glaucoma actually means clouded in Greek. It is one of the most common causes of irreversible blindness throughout the world.

What is Glaucoma?

It is a group of eye diseases usually characterized by increased pressure within the eye and resulting loss of vision. Although the vision loss caused by glaucoma is irreversible, medication or surgery can usually control the pressure within the eye and slow or halt the progression of the disease. If glaucoma is diagnosed in its early stages, it can be treated effectively, and vision can usually be preserved.

What are the various types of glaucoma?

The are 2 major types

  1. Acute glaucoma (also known as angle closure glaucoma)
  2. Chronic glaucoma ( open angle glaucoma) 
Normally the fluid inside the eye (aqueous humor) is produced inside the eye by the ciliary body and flows out into the veins through trabecular meshwork, a sieve-like structure in the angle between the cornea (the transparent layer of tissue at the front of the eye) and the iris (the colored part of the eye). Any abnormality in the outflow of this aqueous humor results in its accumulation inside the eye and hence increased pressure.

What is acute glaucoma?

Acute glaucoma is a medical emergency that develops when pressure inside the eye increases rapidly. Acute glaucoma can develop at any age, and must be treated promptly to prevent permanent vision loss.

What are the symptoms of acute glaucoma?       
     
Patient complains of eye pain, severe headache, nausea, sometimes vomiting also sensitivity to light, redness of the eyes, blurred vision, and seeing rainbow-like halos around lights.
     
What is the treatment of acute glaucoma?      
    
The pressure inside the eye must be lowered within 24 hours to prevent severe vision loss. Medications to decrease the production of aqueous humor and to constrict the pupil are used to treat acute glaucoma. Laser surgery is another common treatment..
    
How can I know if I’m predisposed to have acute glaucoma?        
A test called gonioscopy can be performed as part of an eye exam can predict a person’s chance of having an acute attack.
     
What is Open angle glaucoma?      
     
It is the most commonest type of glaucoma. In this disease the pressure within the eye rises gradually, and due to increased pressure on the nerve of the eye there is vision loss which progresses over a period of years.
     
Am I at risk to develop glaucoma?      
     
If you have a family history of glaucoma (specially in siblings), suffer from diabe-tes, are above 40 yrs of age, then you are at risk to develop glaucoma and you should get your eyes examined routinely by an eye specialist. The incidence of glaucoma increases with age, is more in males and in blacks. Near-sightedness has also been associated with open angle glaucoma.           

What are the symptoms of Open angle glaucoma?       
     
In its early stages, glaucoma is symptom-less. The gradual increase of pressure in-side the eye does not cause any pain or discomfort. As the disease progresses, however, vision begins to deteriorate. The deterioration usually begins with the peripheral vision

It is a tool to measure IOL Power for implantations during cataract surgery. It first measures the axial length of the eye then calculates IOL Power with keratometry readings.  We have immersion mode A-Scan which prevents corneal indentations (which usually occurs in routine A-Scan machines causing error) & hence giving a more accurate reading. It is mainly utilized for two surgical procedures. The use of this laser does not require any anesthesia & recovery period is minimal.       
             
YAG Capsulotomy: YAG laser is used to remove capsular opacification after cataract within seconds. No Pain, No discomfort.
            
YAG PI (Peripheral iridutomy): It is one of the treatment modalities in closed angle glaucoma. With the help of YAG laser an opening is created in the peripheral iris hence bypassing the blocked area & bringing down the pressure in the eye. Further attacks are also prevented.
                 
What is After-Cataract Yag Laser Treatment ?      
     
In cataract surgery the bag of the lens is left inside the eye purposefully to support the IOL (Intra Ocular Lens). Over a period of time, a few remaining cells of the natural bag grow and make the bag thick and opaque which obscure the vision. This is not a complication. With help of the Yag Laser small opening is made in the bag in the centre in just a few seconds. This is not a surgery. Yag Laser Treatment is a noninvasive OPD procedure and requires no anesthesia and no patch. Younger patients are more prone to this thickening of the bag.

Leonardo da Vinci was, the first to suggest the use of contact lenses to correct defective vision in 1508. In Germany, nearly four centuries later, the first contact lens manufactured was a glass shell that covered the entire surface of the eye. Modern contact lenses appeared in the 1940s.    

Today, many people prefer them to eyeglasses for reasons of convenience or cosmetics, although spectacles offer far more protection for the eye. Present-day contact lenses cover only the cornea of the eye; So-called soft lenses, now in common use, are made from a soft plastic material that molds itself to the shape of the cornea.  With advancing technology Contact Lenses can correct all forms of visual defects including Astigmatism safely and comfortably. There are basically two types of contact lenses : Rigid and Soft. Both types come in varying materials with different levels of oxygen permeability. The higher the oxygen permeability the better it is for the eye.  Contact Lens care is the most important part of contact lens wear. Good eye care also includes proper care of the lenses.

All types of contact lenses should always be kept clean, disinfected and hydrated. Lens hygiene is the key to a comfortable wear. All contact lenses accumulate a variety of deposits, from the eye itself whilst wearing them, and from the environment. The environment also contributes towards building up of these deposits especially on soft lenses. The chemical pollution from cars, factories etc., dust, metal in the air get embedded onto the surface, causing harmful contamination on the lens surface, over a period of time. In order to prevent these deposits, all contact lenses should be cleaned, soaked and disinfected daily or disposable should be used. A new system of prescribing and manufacturing contact lenses has been introduced known as the Disposable contact lens, which enables the replacement of lenses on Daily, Weekly, Monthly, and Three Monthly basis.

The advantages of this system of prescribing is that, frequent infections due to protein deposits and environmental pollution are almost eliminated, the eye gets more Oxygen, Vision and Comfort are greatly increased, and best of all, simple cleaning systems like multipurpose solutions can be used safely.

What is Astigmatism?   
   
     
A visual defect caused by an eye surface that is irregularly shaped. People with astigmatism see less clearly at any distance. It is corrected by the help of cylindrical lens.

It is also known as near-sightedness. The image of the objects we see is formed ahead of the retina instead of over it . It is corrected by the help of concave (minus) glasses.

What is Presbyopia ?      
    
There is difficulty in reading and near work. It occurs in people over 40 years of age. It is due to age related weakness of the eye muscles which are used to focus on near objects. It is corrected with the help of convex (plus) glasses.
    
Can eye exercises improve my vision?      
    
You can train your eye muscles to work better together and correct problems like strabismus (crossed eyes) which involves training the muscles that align the eye. Accommodative exercises may also help children who have strain while reading and writing.
    
Can my eyes get sunburned?      
    
Yes ! "snow blindness" and a welder's flash are examples of "sun burned" eyes. Outdoor enthusiasts and those with sensitive eyes should invest in sunglasses with an ultraviolet filter to block harmful UV radiation from the sun. Contact  Lens wearers are especially sensitive to bright light, they should always wear a good pair of non-prescription sunglasses with their contacts when they are outside.
