Where expert science is applied for all our product with scientific objectivity from the field to technical testing in our laboratory facility We do investigation whenever an OOS test results is obtained. The purpose of the investigation is to determine the root cause of the OOS which could be identified either as an aberration of the measurement process or an aberration of the manufacturing process. We do assign the two phases of investigation: Phase I is an initial assessment of the accuracy of the lab data. This should be done before test preparations are discarded. If the Phase I assessment indicates that no meaningful errors were made in the analytical method, a full-scale investigation (Phase II) we are conducting. Responsibilities of laboratory personnel An analyst is responsibility for achieving accurate lab testing results. The lab supervisor involved as soon as an OOS is identified. The supervisor discussing the test method with the analyst; confirm the analyst knowledge, raw data, equipment and reagents performance, and performance of the correct procedure. Lab management made alert for developing trends, such as atypical results, before OOS results are obtained. We perform additional testing that may be necessary and be cautious. Clearer definitions on “Re-testing”, “Re-sampling”, “Averaging”, and “Outlier Tests” were defined in our procedure. Expansion of investigation to outside the laboratory A full scale investigation (Phase II) were carried out this include a review of production and sampling procedures, and will often include additional lab testing. The investigational team, include QA, QC, manufacturing, process development, maintenance and engineering. Team documents the assessment of the expanded investigation on the OOS report. Final evaluation of all test results Our Quality Unit is responsible for interpreting the investigation results. The results should be evaluated, and the batch quality and release decision determined and documented quality assurance.
