The underlying tissues that keep your skin looking youthful and plumped up begin to break down, often leaving laugh lines, smile lines, crow's feet or facial creases over the areas where this muscle movement occurs.

Soft-tissue fillers - most commonly injectable hyaluronic acid, collagen or fat - can help fill in these lines and creases, temporarily restoring a smoother, more youthful-looking appearance. When injected beneath the skin, these fillers plump up creased and sunken areas of the face. They can also add fullness to the lips and cheeks. They are also used for peri-orbital rejuvenation, cheek and chin augmentation, nose shaping, correction of crooked nose and depressed nose, scar treatment, acne scar reduction, etc. Injectable fillers may be used alone or in conjunction with a resurfacing procedure, such as a laser treatment, or a re-contouring procedure, such as a facelift.

Women whose breasts have drooped - after breast-feeding, weight loss or through the normal ageing process - usually need a breast lift to eliminate excess skin, plus an implant to provide volume. However, when there is sufficient tissue, Dr. Viral Desai combines the internationally-renowned procedures of Botti and Graf with some of his own techniques, to create a natural augmentation.

Botti’s technique uses the lower part of the breast as an "auto implant", combining it with a tightening of the skin that results in a scar around the areola and a vertical scar in a lollipop configuration. Graf’s procedure is similar, but involves an s-shaped scar with the lower part of the "s" in the outer half of the crease beneath the breast. Dr. Desai’s development reduces the scarring. He uses the concept of keeping the lower part of the breast as an "auto implant" and combines a lollipop closure with a natural-looking "purse-string" gathering of the skin around the areola.

Dr. Viral Desai calls his surgery “Auto-Augmentation”. To provide fullness where it is needed - above the nipple - he moves the gland from the lower part of the breast to the upper area. Endoscopy is a surgical technique that involves the use of an endoscope, a special viewing instrument that allows a surgeon to see images of the body's internal structures through very small incisions.

Of all the cosmetic procedures that use endoscopy, forehead lift is the one which plastic surgeons more commonly perform. Instead of the usual ear-to-ear incision, three or more "puncture-type" incisions are made just at the hairline. Also, bleeding, bruising and swelling may be significantly reduced. With the endoscopic approach, you may recover more quickly and return to work earlier than if you had undergone open surgery.

Endoscopic surgery may also allow you to avoid an overnight hospital stay. Many endoscopic procedures can be performed on an outpatient basis under local anesthesia with sedation. Be sure to discuss this possibility with your doctor. In endoscopic surgery, a probe with a tiny camera transmits images inside the body to a video monitor.

SPECIAL CONSIDERATION AND RISKS
It is important to remember that the endoscopic approach has only recently been applied to plastic surgery procedures. There are some known risks, which vary in severity depending on the procedure being performed. These include infection, fluid accumulation beneath the skin (which must be drained), blood vessel damage, nerve damage or loss of feeling, internal perforation injury, and skin injury.

Patients who tend to be the best candidates for cosmetic endoscopic procedures are those who don't have large amounts of loose hanging skin.
