The knee joint is made up of a shock-absorbing cartilage called meniscus. They exist two menisci, placed between the shin bone and thigh bone. The outer meniscus is called lateral meniscus while the meniscus in the inner side is known as medial meniscus.

Causes:

Sport players are often in the danger of meniscus tear. Quick turning or twisting of knee, heavy weight lifting and accidents are the common causes of meniscus damage. Aging is another factor which enhance the risk of meniscus tear. With age, the cartilage begins to wear down and there occurs lack of blood supply. Thus the meniscus are damaged.

Symptoms:

In some patients, the initial stages do not show any symptoms. They develop signs of meniscus tear at the later stages. Some of the common symptoms are:

  • A clicking sound at the time of injury
  • Knee joint locking
  • Pain during knee movement
  • Swelling and stiffness
  • Limited knee movement

Treatments:

The knee joint x-ray or MRI scan is done, prior to treatment, to detect the type and area of meniscus tear. Another test called McMurray is also sometimes done to confirm the diagnosis of meniscus tear. Both non-surgical and surgical treatment methods exist for treating meniscus tear.

Non-surgical Treatments/Home Treatments

For mild symptoms, it is sufficient to use non-surgical treatment like Home remedies. The non-surgical method of treatment includes:

  • Rest

A short break from activities that may harm the injury or increase the symptoms is recommended.

  • Ice

Applying ice packs on the affected helps to relief pain and swelling. It should be used several times in a day for at least 20 minutes.

  • Compression

Wearing a compression band over the injured part is a good option. It prevents additional blood loss and swelling.

  • Elevation

Resting the leg up higher than the heart is useful to reduce swelling.

  • Non-steroidal anti-inflammatory medicines

Anti-inflammatory medicines aim to reduce pain and swelling in the injured are. Aspirin and ibuprofen are the commonly used medicines.

Surgical Treatments:

In severe cases, when non-surgical methods fail, surgery becomes necessary. The commonly used surgical procedures are:

  • Arthroscopic Meniscectomy

The arthroscopic meniscectomy aims to treat only the damaged part of meniscus. In this procedure, a small camera along with surgical instruments is inserted through a tiny incision made on the knee. This gives a clear inside view of the injured area. The instruments help in the removal of torn part of meniscus. The healthy part of meniscus is left unaffected. The recovery in this surgery is usually fast as compared to other methods.

  • Meniscus Repair

In this surgical procedure, the torn part of the meniscus is repaired. The repaired meniscus provides the lost anatomy of the knee. However, this procedure has some disadvantages such as larger recovery time and limited blood supply to meniscus. Thus meniscus repair is not possible for every person.

  • Meniscus Transplantation

In people suffering with persistent knee pain, the meniscus transplantation surgery is chosen. During surgery, the damaged meniscus is replaced with a healthy meniscus from the donor. The entire meniscus is removed in this treatment procedure.

A group of four muscles are attached to the upper region of shoulder blade through a pair of tendons. These tendons in altogether are known as rotator cuff. These muscles and tendons are responsible for proper rotation and lifting of arm. Any damage to the tendons separate it from the bone. This is known as rotator cuff tear.

Treatments:

The severity of the condition of rotator cuff describes the type of treatment needed. Basically non-surgical and surgical type of treatment procedure exists. Non-surgical treatments are beneficial because they avoid the risk of infection, reaction to anesthesia, and permanent stiffness. Also it requires less recovery time. However, some it accompanies some disadvantages such as no muscle strength improvement, increase in tear size, or limited daily activities.

  • Non-surgical Treatment

About 50% of patients finds improvement in pain and functionality through non-surgical method. However, shoulder strength is not regained through this procedure. The options included in non-surgical treatment are:

  • Rest:

In acute symptom condition, proper rest is essential. Rest helps the shoulder inflammation to improve. Sling is sometimes preferred to hold still and protect the shoulder. However, complete immobilization of shoulder is not recommended as it may cause frozen shoulder. Simple exercises which do not strain the rotator cuff are favored.

  • Activity Modification:

Avoid activities which creates stress on the rotator cuff and hence increase pain. Such activities include heavy lifting, vigorous activities, irregular sleeping position, etc.

  • Non-steroidal ant-inflammatory medication:

The control of symptoms is greatly possible with the intake of anti-inflammatory medications. Pain and swelling in affected areas is reduced with these medications. Naproxen and ibuprofen are the commonly used medications. These medications are often prescribed, under the supervision of a doctor, for a short period of time.

  • Physical therapy and strengthening exercises:

For proper treatment of rotator cuff injury, it is essential to have strengthened muscles. Physical therapy is a good option to restore lost strength and mobility of the muscles. It includes exercises which are simple and prevent recurrence of shoulder pain. Stretches help to improve range of motion and reduces any discomfort to shoulders.

  • Steroid injection:

Steroid injection such as cortisone is useful to limit the symptoms of rotator cuff tear. Injections provide temporary relief from pain and inflammation. So it is recommended to use physical therapy after injections to prevent chances of occurrence of shoulder discomfort.

  • Surgical Treatment

In case, the symptoms are not improved with non-operative methods, then surgery is opted. Surgery is also useful to treat tears of more than 3cm in size. The surgery helps to retain the original function of the shoulder. The surgery involves connecting the torn tendons back to the arm bone(humerus). This process is also known as rotator cuff repair. This can be done using three types of surgical method:

  • Arthroscopic surgery:

In this type of surgery, the person creates small incisions on the shoulder muscles. Through these incisions, a small tube with camera and tiny surgical instruments attached at one end are inserted. This tube is known as arthroscope and is used for rotator cuff repairing.

  • Open surgery:

This method makes use of normal surgical instruments to perform surgery. These instruments are inserted through a large hole made in the shoulder muscles.

Osteoarthritis is a common disorder of joints. With aging, the cartilage becomes thin and hence breaks down. This limits the joint motion and causes intense pain. Osteoarthritis mainly affects the joints of hands, knees, hips, and toes. It often worsens with time and has no permanent cure. Middle or old aged people are commonly affected.

The actual cause of osteoarthritis is not clear. Researches show that wear-and-tear of bones often result in osteoarthritis. It may occur due to aging. This causes the deterioration of cartilages. As the cartilage is damaged, the covering of bones is removed. The bones start to rub against each other. This is assumed to be the cause of osteoarthritis. Genetic factors, overweight, fractures, etc can worsen the condition.

Clubfoot is a foot and ankle deformity mainly present from birth. Clubfoot condition is characterized with inward twisted foot and downward toes.

Treatments

Clubfoot may be detected prior to birth by an ultrasound. After birth, the appearance and movement of feet help to confirm the diagnosis. Bone position and alignment are determined through x-rays. The early treatment involves non-surgical procedure. In severe cases, surgery is chosen.

  • Non-surgical Treatment

For best results, the treatment should begin in right way. Clubfoot is normally treated without surgery. The non-surgical treatment procedure includes

  • Ponseti Method (stretching and casting)

This is the most common and successful treatment for treating clubfoot. It is usually started in the first few weeks of the childbirth. During treatment, the foot of the patient is kept inside a cast. This cast covers the body from toes to upper thigh. The old cast is replaced with a new one in every 4-7 days. By the time the last cast is used, the heel cord of patient is cut down. This is done to get a normal length of the heel cord. The recovery in this method is usually in 2-3 months. Physical therapy along with special braces or shoes should be used for a few years.

  • French Method (stretching, splinting and taping)

Generally, this treatment method is started soon after birth. In this method, the foot is stretched to the correct position. Tapes or splints are used to hold the foot in this correct position. The usual duration of treatment is 6 months. The splints or tapes are changed every day. Until complete recovery, night splints and special stretching exercises are recommended.

  • Surgical Treatment
Occasionally, the non-surgical treatment options fail. In such conditions, surgery is chosen as an option. The surgery is often performed at the age of 9-12 months. The surgery corrects the abnormalities in ligaments, tendons or joints. For this, the position of tendons is changed to get a better functional tendon. Thus, a longer heel cord is achieved which gives a balanced foot.

A total hip replacement is basically a surgery where artificial materials are used to replace a damaged hip. A chronic and inflammatory hip often requires surgery. The surgery is opted when the conservative methods fail to treat the painful condition. With time, these replaced parts usually deteriorate. So surgery is opted as a last resort for treatment.

Prior to the surgery, general anesthesia is given to the patient. It numbs the area to be operated. So the person does not experience pain or any other discomforts during surgery. During hip replacement surgery, an incision is made along either lateral or posterior or anterior part of the hip. Certain surgical instruments are then inserted through this incision. Using these instruments, the the arthritic or damaged bone is removed. The empty space is replaced with a prosthesis (artificial materials). There exists different types of prosthesis. It may be metal-on-plastic, ceramic-on-ceramic, or metal-on-metal type of prosthesis. Stitches or sutures are used to close the incisions. Before incision closure, tubes are inserted to collect the draining blood. The duration for surgery is 1-2 hours. Braces or casts are also used to support the hip. The results of hip replacement surgery usually last for 10-15 years.

In case of severe damages to knee, a knee replacement surgery is chosen. This surgery aims to replace the torn knee with an artificial joint. In total replacement knee surgery, both sides of knee joint are surgically treated. It is a major surgery and hence is recommended only if other methods fail. Mostly people in between age of 60-80 years are surgically treated as they are physically little weaker than young people.

A proper examination of the person is done to determine the best anesthesia. This avoids any reaction to anesthesia. The anesthesia used may be spinal epidural, or general anesthesia, or regional nerve block anesthesia. The surgery is usually over in 1-2 hours. In this procedure, an incision is made on the knee to remove the damaged part. The damaged bone and cartilage on both side of the knee are cleared out. Implants made of plastic and metal are used to replace the damaged parts. These implants help to re-align the the torn knee. Thus a healthy and functionally improved knee is achieved. Stitches or staples are used for covering the wound.
