How does CT work?

CT uses X-rays to perform its work. These X-rays come from an X-ray tube which is placed inside a round machine called the gantry. The part of your body to be scanned goes inside a hole in this gantry.


Why is CT such a good modality?

Ordinary X-ray pictures are one-dimensional representations of body parts but CT allows us to section the body to view its inner parts. Thus, the skull X-ray only shows us the outer part, whereas CT shows us the inner parts of the brain.

Where is CT useful?

All parts of the body can be scanned with CT. For each part, there are specific indications or reasons for scanning. Your referring doctor and the consultant radiologist are the best judges of the usefulness of CT scanning in a specific situation and areas.

Are there any dangers of CT scanning?

Though X-rays involve radiation, there are no dangers, in practice. In women who are pregnant, however, CT scanning should be done after weighing all the risks.

What are the standard instructions that the patient should follow?
  • Please get all the old X-rays, Sonography, CT and MR films along with the other papers, operative notes, discharge cards, etc. relevant to the case.
  • Please come fasting for at least six hours, prior to the scan.
  • There should, preferably be an accompanying friend or relative.
  • Please inform the doctor, nurse or the receptionist, if you are at high risk for "dye" injection, as described above, i.e. if you have a history of drug reactions, bronchial asthma, cardiac or kidney disease, etc.
  • Please inform the doctor, nurse or the receptionist if you are pregnant or think you may be pregnant

A Doppler ultrasound test uses reflected sound waves to evaluate blood as it flows through a blood vessel. It helps doctors evaluate blood flow through the major arteries and veins of the arms, legs, and neck. It can show blocked or reduced blood flow through narrowing in the major arteries of the neck that could cause a stroke. It also can reveal blood clots in leg veins (deep vein thrombosis, or DVT) that could break loose and block blood flow to the lungs (pulmonary embolism). See pictures of a stroke and an embolus . During pregnancy, Doppler ultrasound may be used to look at blood flow in an unborn baby (fetus) to check the health of the fetus.

Color Doppler uses standard ultrasound methods to produce a picture of a blood vessel. Also, a computer converts the Doppler sounds into colors that are overlaid on the image of the blood vessel and that represent the speed and direction of blood flow through the vessel.

Why It Is Done
  • Detect blood clots and blocked or narrowed blood vessels in almost any part of the body, especially in the neck, arms, and legs. Blocked or narrowed arteries of the neck can cause dizziness, loss of vision, paralysis, weakness, numbness, or other symptoms of a stroke.
  • Evaluate blood flow after a stroke or other condition that might be caused by a problem with blood flow. Evaluation of a stroke can be done through a technique called transcranial Doppler (TCD) ultrasound.
  • Evaluate abnormal veins causing varicose veins or other problems.
  • Find out the amount of blood flow to a transplanted kidney or liver.
  • Monitor the flow of blood following blood vessel surgery.

How To Prepare

You may be asked to not use products that contain nicotine (cigarettes, chewing tobacco) for 30 minutes to 2 hours before the test. Nicotine causes blood vessels to constrict and may give false results.

How It Is Done

This test is done by a doctor who specializes in performing and interpreting imaging tests (radiologist) or by an ultrasound technologist (sonographer) who is supervised by a radiologist. It is done in an ultrasound room in a hospital or doctor's office.

You will need to remove any jewelry that might interfere with the Doppler ultrasound scan. You may need to take off all or most of your clothes, depending on which area is being examined (you may be allowed to keep on your underwear if it does not interfere with the test). You will be given a cloth or paper covering to use during the test.

  • For abdominal scans, you will lie on your back.
  • For chest scans, you will lie on your back with your neck slightly extended.
  • For head and neck scans, your head may be turned to one side.
  • For an arm or leg scan, your head will be slightly raised and the exposed arm or leg will be turned slightly outward. Sometimes for a leg scan, you may be asked to lie on your stomach.
  • During pregnancy, you will lie on your back or on your left side with your belly exposed.

Gel is applied to the skin to promote the passage of the sound waves. The transducer is placed in the gel and moved along the skin. You need to lie very still during the procedure. You may hear sounds that represent the flow of blood through the blood vessels.

How It Feels

There is normally no discomfort involved with having a Doppler ultrasound test. The gel may feel cold when it is put on your skin unless it is first warmed to body temperature. If your blood pressure is taken during the test, you will feel pressure when the blood pressure cuff is inflated.

The purpose of this study is to determine the size of your heart, to evaluate how well your heart is functioning or pumping and to assess the structure and function of the valves within the heart. A 2-D (or two-dimensional) echocardiogram is capable of displaying a cross-sectional "slice" of the beating heart, including the chambers, valves and the major blood vessels that exit from the left and right ventricle.

A Doppler echocardiogram measures the speed and direction of the blood flow within the heart. It screens the four valves for leaks and other abnormalities. By assigning color to the direction of blood flow, (Color Flow Mapping), large areas of blood flow may be studied. These color flow mappings allow abnormal blood flow characteristics to be interpreted by the cardiologist.

What will happen during the test

The test will take about 45 minutes. You will be asked to remove your shirt or blouse and put on a gown or a sheet to keep you comfortable and maintain privacy. After lying on an examination table, the technician will apply a colorless gel to your chest. The transducer will be moved back and forth across your chest to obtain several views of your heart.

You may be asked to move from your back and to the side. Instructions may also be given for you to breathe slowly or to hold your breath. This helps in obtaining higher quality pictures. With Doppler echocardiograms, as the transducer moves over your heart, you will hear a "whooshing" sound, much like that of a washing machine. This sound relates to the movement of blood within your heart chambers.
The images are constantly viewed on the monitor and recorded for a permanent record of the examination. This is reviewed by the physician prior to completion of the final report.


What is the preparation for the test?

Other than wearing loose-fitting clothing, there are no special restrictions or preparation prior to a 2D Doppler echocardiogram. Although the gel is water soluble and should not stain; it is messy and could get on your clothing so please be aware and dress appropriately.
