Ceftazidime is a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, particularly those caused by Gram-negative organisms. It is often used in hospital settings to treat severe infections.

Indications

Ceftazidime injection is indicated for the treatment of:

  • Lower respiratory tract infections
  • Skin and skin structure infections
  • Urinary tract infections (complicated and uncomplicated)
  • Bacterial septicemia
  • Bone and joint infections
  • Gynecologic infections
  • Intra-abdominal infections, including peritonitis
  • Central nervous system infections, including meningitis
  • Empirical treatment of febrile neutropenia
Mechanism of Action

Ceftazidime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), disrupting the formation of the bacterial cell wall, which leads to cell lysis and death.

Dosage and Administration

Ceftazidime is administered by intravenous (IV) or intramuscular (IM) injection. The dosage depends on the type and severity of the infection, as well as patient factors such as age and kidney function.

Adult Dosage:

  • Typical dosage: 1 to 2 grams every 8 to 12 hours.
  • Severe infections, including those caused by Pseudomonas aeruginosa: Up to 2 grams every 8 hours.

Pediatric Dosage:

  • 30 to 50 mg/kg every 8 hours, with a maximum of 6 grams per day.
  • For neonates (0 to 4 weeks): 30 mg/kg every 12 hours.

Dosage Adjustments:

  • Patients with renal impairment require dosage adjustment based on creatinine clearance levels.
Side Effects

Common side effects include:

  • Injection site reactions (pain, inflammation)
  • Diarrhea
  • Nausea and vomiting
  • Rash
  • Pruritus (itching)
  • Headache

Serious side effects may include:

  • Severe allergic reactions (anaphylaxis)
  • Clostridium difficile-associated diarrhea
  • Seizures (particularly in patients with renal impairment)
  • Hemolytic anemia
  • Thrombocytopenia (low platelet count)
  • Hepatotoxicity
Contraindications and Precautions
  • Known hypersensitivity to ceftazidime, cephalosporins, or any component of the formulation.
  • Use with caution in patients with a history of penicillin allergy, as cross-reactivity may occur.
  • Monitor renal function, particularly in elderly patients and those with pre-existing renal impairment.
  • Use with caution in patients with a history of gastrointestinal disease, particularly colitis.
Drug Interactions
  • Aminoglycosides: Concurrent use with aminoglycoside antibiotics increases the risk of nephrotoxicity. Renal function should be monitored.
  • Diuretics: High-dose diuretics such as furosemide can also increase the risk of nephrotoxicity when used concurrently with ceftazidime.
Special Considerations
  • Pregnancy: Classified as Category B. Animal studies have not shown any harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed.
  • Breastfeeding: Ceftazidime is excreted in human milk in low concentrations. Caution should be exercised when administering to a nursing woman.
  • Elderly Patients: Dose adjustment may be necessary due to age-related decline in renal function.
  • Pediatric Use: Generally safe for use in children, with dosage adjustments based on age and weight.
Storage
  • Store vials at 20°C to 25°C (68°F to 77°F).
  • Reconstituted solution should be used immediately or stored according to the manufacturer's guidelines, typically within 12 to 24 hours if refrigerated.
