Isavuconazole is an antifungal medication available in capsule form, used primarily for the treatment of serious fungal infections. Here is a detailed overview of isavuconazole capsules:
Uses:
Invasive Aspergillosis:
Treatment of invasive aspergillosis, a serious infection caused by Aspergillus species, particularly in immunocompromised patients.
Invasive Mucormycosis:
Treatment of invasive mucormycosis, also known as zygomycosis, caused by fungi in the order Mucorales. This is a rare but severe infection.
Mechanism of Action:
Isavuconazole is a triazole antifungal that works by inhibiting the enzyme lanosterol 14-alpha-demethylase, which is crucial for converting lanosterol to ergosterol. Ergosterol is a key component of the fungal cell membrane. Inhibition of its synthesis disrupts the cell membrane's integrity, leading to cell death.
Administration:
Dosage: The recommended initial loading dose is 372 mg (equivalent to 200 mg of isavuconazonium sulfate) every 8 hours for 48 hours. This is followed by a maintenance dose of 372 mg once daily.
Instructions: Isavuconazole capsules can be taken with or without food. Swallow the capsules whole without crushing, chewing, or opening them.
Side Effects:
Common:
Nausea
Vomiting
Diarrhea
Headache
Abdominal pain
Serious:
Hepatotoxicity (liver toxicity)
QT interval shortening (a type of heart rhythm change, unlike other azoles that usually prolong the QT interval)
Severe allergic reactions
Infusion-related reactions (when given intravenously)
Hypokalemia (low potassium levels)
Monitoring:
Patients receiving isavuconazole should be monitored for:
Liver function tests (LFTs)
Electrolyte levels (potassium, magnesium)
Signs and symptoms of heart rhythm changes
Precautions:
Drug Interactions: Isavuconazole can interact with several medications, including certain immunosuppressants, anticoagulants, and other drugs metabolized by the CYP3A4 enzyme. It is important to inform healthcare providers of all medications being taken.
Renal and Hepatic Impairment: While no dose adjustment is necessary for renal impairment, caution is advised in patients with severe hepatic impairment.
Pregnancy and Breastfeeding: The safety of isavuconazole in pregnant or breastfeeding women has not been established, so it should be used only if the potential benefits justify the potential risks.
