What are Cephalosporins?

They bind to and block the activity of enzymes responsible for making peptidoglycan, an important component of the bacterial cell wall. Since the first cephalosporin was discovered in 1945, scientists have been improving the structure of cephalosporins to make them more effective against a wider range of bacteria. Each time the structure changes, a new "generation" of cephalosporins are made. So far there are five generations of cephalosporins. All cephalosporins start with cef, ceph, or kef. Note that this classification system is not used consistently from country to country.


What are cephalosporins used for?

Cephalosporins may be used to treat infections caused by susceptible bacteria, such as:

MSD has launched a new intravenous antibiotic in the UK capable of treating certain Gram-negative bacterial infections, offering a new weapon in the fight against antimicrobial resistance.

Zerbaxa is a combination product containing ceftolozane, a cephalosporin antibacterial drug that kills bacteria by interfering with the production of certain molecules essential to building their protective cell walls, and tazobactam, which blocks the action of certain enzymes called beta-lactamases, whih enable bacteria to break down beta-lactam antibiotics like ceftolozane, making them resistant to the antibiotic’s action.

The drug is now available in the UK to treat complicated intra-abdominal infections, acute pyelonephritis and complicated urinary tract infections  in hospital settings, offering an alternative to current standard of care with carbapenems, to which there is currently growing antimicrobial resistance.

The move follows European approval back in September, on the back of data showing Zerbaxa to be at least as effective as other antibiotics in curing infections. 

In one trial involving 1,083 patients who mostly had kidney infection or in some cases a complicated urinary-tract infection, the drug successfully treated the infection in about 85% of the cases, compared with 75% of those treated with the antibiotic levofloxacin. In a second study involving 993 patients with complicated intra-abdominal infections, both Zerbaxa and another antibiotic, meropenem, were able to cure around 94% of patients.

Antimicrobial resistance is one of the biggest threats facing public health in the 21st century. The Department of Health has estimated that AMR already costs the European Union at least 1.5 billion Euros per year and claims around 25,000 lives - with estimated 3,000 in the UK. But experts are forecasting that the annual death rate in Europe will rocket to 390,000 by 2050, and from 700,000 to 10 million, globally, highlighting the urgent need for new antibiotics.

MSD picked up the antibiotic through its $9.5-billion purchase of Cubist Pharmaceutical earlier this year.

Description:

Ticarcillin is a carboxypenicillin. It is almost always sold and used in combination with clavulanate as ticarcillin/clavulanic acid. Because it is a penicillin, it also falls within the larger class of beta-lactam antibiotics. Its main clinical use is as an injectable antibiotic for the treatment of Gram-negative bacteria, particularly Pseudomonas aeruginosa. It is also one of the few antibiotics capable of treating Stenotrophomonas maltophilia infections.

 

It is provided as a white or pale-yellow powder. It is highly soluble in water, but should be dissolved only immediately before use to prevent degradation.

 

Mechanism of action:

Ticarcillin's antibiotic properties arise from its ability to prevent cross-linking

of peptidoglycan during cell wall synthesis, when the bacteria try to divide, causing cell death.

 

Ticarcillin, like penicillin, contains a β-lactam ring that can be cleaved by beta-lactamases, resulting in inactivation of the antibiotic. Those bacteria that can express β-lactamases are, therefore, resistant to β-lactam antibiotics. Due, at least in part, to the common β-lactam ring, ticarcillin can cause reactions in patients allergic to penicillin. Ticarcillin is also often paired with a β-lactamase inhibitor such as clavulanic acid (co-ticarclav).

 

Other Uses:

In molecular biology, ticarcillin is used to as an alternative to ampicillin to test the uptake of marker genes into bacteria. It prevents the appearance of satellite colonies that occur when ampicillin breaks down in the medium. It is also used in plant molecular biology to kill Agrobacterium, which is used to deliver genes to plant cells.

 

Dosing and administration:

Ticarcillin is not absorbed orally, so must be given by intravenous or intramuscular injection.

 

Trade names and preparations:

·         Ticarcillin: Ticar was formerly marketed by Beecham, then SmithKline Beecham until 1999, when it merged with Glaxo to form GlaxoSmithKline; it is no longer available in the UK. US distribution ceased in 2004. Ticar was replaced by Timentin.

 

However Timentin contains clavulanate unlike Ticar

For people at high risk of serious flu complications, early treatment with an antiviral drug can mean having milder illness instead of more severe illness that might require a hospital stay.


When should antiviral drugs be taken for treatment?

Studies show that flu antiviral drugs work best for treatment when they are started within two days of getting sick. However, starting them later can still be beneficial, especially if the sick person is at high risk of serious flu complications or is in the hospital with more severe illness.  Follow instructions for taking these drugs.

What antiviral drugs are recommended this flu season?

There are four FDA-approved antiviral drugs recommended by CDC to treat flu this season.

  • oseltamivir phosphate (available as a generic version or under the trade names Tamiflu® and Antiflu®),
  • zanamivir (trade name Relenza®)
  • peramivir (trade name Rapivab®), and
  • baloxavir marboxil (trade name Xofluza®).

Generic oseltamivir external icon and Tamiflu® are available as a pill or liquid suspension and are FDA approved for early treatment of flu in people 14 days and older. Zanamivir is a powder that is inhaled and approved for early treatment of flu in people 7 years and older. It is administered using an inhaler device and is not recommended for people with breathing problems like asthma or COPD.) Oseltamivir and zanamivir are given twice a day for 5 days. Peramivir is given once intravenously by a health care provider and is approved for early treatment of flu in people 2 years and older. Baloxavir is a pill given as a single dose by mouth and is approved for early treatment of flu in people 12 years and older. It is not recommended for pregnant women, breastfeeding mothers, outpatients with complicated or progressive illness, or hospitalized patients because there is no information about use of baloxavir in these patients.)

How long should antiviral drugs be taken?

To treat flu, oseltamivir or inhaled zanamivir are usually prescribed for 5 days, or one dose of intravenous peramivir or oral Baloxavir for 1 day. Oseltamivir treatment is given to hospitalized patients, and some patients might be treated for more than 5 days.

  What are the possible side effects of antiviral drugs?

Side effects vary for each medication. The most common side effects for oseltamivir are nausea and vomiting. Zanamivir can cause bronchospasm, and peramivir can cause diarrhea. Other less common side effects also have been reported.
